What do you mean by headphone output? Most of the time you are sending audio to the main L/R mix which is monitored by default. Go to the settings and open Audio Outputs. Select X-Air ASIO and proper USB audio return channels. Now you should be able to hear audio from vMIX in your headphones.

With XR18, I have assigned vMix headphone output to two XR18 input channels (x+y). Then in X Air mixer, those channels are selected to the channel strip by changing inputs from mic to usb x and y as a stereo pair. If I wan to monitor vMix through headphones connected to XR18, I just solo those the headphone return channels in X Air.

It's not an entirely straightforward configuration but I hope this helps for your problem.
