Ok after working on this for two days I finally figured this out.
I searched the docs, every FB group and post on this forum and it is not mentioned anywhere (I could find).

Problem:
When using the SelectTitlePreset shortcut, values 1 and 2 do not select the first and second title.

Solution:
The first title value MUST be 0 and the second title value is considered 1.

As a ex-programmer I get it, this is a typical binary approach. But for humans we would consider the first title to be #1 :)

Maybe this is documented some where and I missed it. Or maybe it is my configuration but either way this is not obvious.
