Fullscreen vertical / portrait desktop

Hi all,
I have been exploring vertical output but I can't quite seem to make it work for my purposes, wondering if anybody has some insight.
I have a display, a monitor output on the same machine that vMix is installed to. The orientation of this display is physically in portrait at 2160x3840, and in Windows the display resolution is natively 2160x3840 with the display orientation set to portrait.
I see that the vertical settings in vMix are more or less tripling the width and outputting a landscape image, which then I suppose is cropped by an endpoint like Instagram. Do I have this right?
I have tried using the custom Position settings under Settings > Display, but it doesn't seem to allow anything wider than the desktop resolution. The image height is correct, but the width is squished leaving the center 'vertical' area of the image skewed.
Is there another way I should be approaching this? Am I just out of luck?
For other software considerations and use cases, the desktop must remain in a portrait orientation.
