Has anyone been able to get a BM Duo 2 card to input 3 SDIs and output 1 SDI? I am only able to input 2 (at a time). BM Tech support blames the software, as it seems to work on their MediaExpress software (using a clip for playback).
I am trying to input 3 cameras and use the 4th port as External (port 4 IDs as Deckling Video Render 5).

Yes. You need to set it up with the BM Desktop Utilty tool. Read their user manual for instructions. It is quite simple.

Set in order: SDI1, SDI3&4, SDI2. Last will be output. First is ref.

I realize that this is a "mapping" issue. I also know that the card appears to re-map itself whenever the PC reboots. My point here is to create a standard so operators can set this card up from scratch. I have been unable to find a guide anywhere that relates this card to vMix, and have heard a lot of complaints...
Here are two maps. Note that the biggest difference is the External Output filename/device. I also had a "not found" issue adding more than 2 cameras/sources when attempting to set more than 2 cameras as 720, so I set the remaining (3rd) one as 1080.

Yes you can, I own two of these cards myself. Just set them all up as in/outputs, 1 to sdi 1, 2 to sdi 3, 3 to sdi2, 4 to sdi 4. Standard you get them configured as 2 channel with loop out, so you need to change that in the control panel at the BMD utility.
Now you can setup 3 inputs in vMix, and setup external output to use the fourth.
Please keep in mind that the connections are not 1 to 4. If you look at the card standing up it is like this:
