A cilindrical cutout ( transparent) in the background image with a 'video' placeholder behind it or just a cilindrically cropped UV-map on top of it all ?

Check basic multiview or other virtual set codes for ideas

Can be done with Color Key , Key/Fill option.
Just a matter of making the black and white mask in your favorite graphics program
I can expand further if needed

As stated above, it is technically possible, just not natively within vMix and it's current tool set.
You'll need to 'build' your own masked shape to use as a special effects overlay. This could be done with a program like PhotoShop or Fireworks or any graphic design program that can render an alpha channel.

Alpha channel masking is the ultimate answer to complex crops. Create whatever shape you like in Photoshop, use it as a mask in vMix. This technique is decades old and well proven.
